Need to report against the MYSQL db

I need to create a report that will list all of the cases i have and then tell me who are the custodians for those cases. I need this to be a one page report so I can get a heads up view of all of the legal hold activity in one shot.  I am able to connect to the MYSQL db but I can't figure out how to proceed beyond that.  Any help on this would be greatly appreciated.
